Integrated Care Pathways
The Care Pathways module defines a preferred, predetermined route or pathway for a patient's care or treatment. Actions which require a predetermined sequence or schedule can be planned into a workflow. As each action is performed it may then be recorded and reported on. The Care Pathway module is able to cross multiple settings and organisations, with the Chart View shared by multiple carers.
Care Pathways works with the Workflow Application Layer (WAL) within Case Notes and provides a way to construct, assign and monitor information about care and treatment for each patient. It provides a graphical view that shows progress of patient care at a glance. Care Pathways is also be used to trigger other Case Notes modules such as Smart Forms for clinical data input.
Actions in Case Pathways can be scheduled to be fixed or flexible, with the option to complete them in parallel or in advance of specific points in the workflow. A patient can also have multiple Pathways for multiple conditions, with common management of actions.
