Care Planning

The Care Planning module provides structured care plans that are used to direct and monitor all health and social care needs, actions, and responsibilities relating to a patient’s care. Designed by experienced users and specialists from Health and Social Care, the Care Planning module is quick to learn and simple to use. Care Plans are built from an extendable library of options and are monitored and updated using on-screen forms.

 

Care Planning

Care Plans exist for the benefit of the patient, and should therefore be based around their needs. Each plan summarises identified needs and how they are to be met - it is a formal record setting out what is going to be done, why, when, and by whom.
