Long Term Conditions
The Long Term Conditions (LTC) package provides support for Long-Term Conditions, Chronic Disease Management and National Service Frameworks (NSF's) including,

It is implemented using elements from Case Notes’ Smart Forms, Care Pathways and Care Planning modules which can be locally configured to meet the exact requirements of any NSF.
The Smart Forms concept provides the building blocks for inter-disciplinary and multi-agency Shared Care, required to support the NSF agenda and the Single Assessment Process (SAP).
It is particularly important that an LTC package can manage multiple or complex conditions, avoiding the repetition of tasks which are needed by two conditions and providing clarity in presenting information. Case Notes’ LTC package uses Care Pathways management to manage complex conditions.
