Packages - Long Term Conditions

Case Notes packages provide a licensed package of functionality across core services and modules, pre-configured for a specific specialisation.

Overview

The Long-Term Conditions package provides support for Long-Term Conditions, Chronic Disease Management and National Service Frameworks (NSF's). These include Coronary Heart Disease; Cancer; Older People; Diabetes; Mental Health; Renal Services and the Neurological LTC. It is implemented using elements from Case Notes’ Smart Forms, Care Pathways and Care Planning modules which can be locally configured to meet the exact requirements of any NSF.

The Smart Forms concept provides the building blocks for inter-disciplinary and multi-agency Shared Care, required to support the NSF agenda and the Single Assessment Process (SAP).

The Case Notes NSF module includes standards-based NSF data sets and provides libraries of NSF forms for a complete solution.

It includes forms for-

Neurological LTC

It is particularly important that an LTC package can manage multiple or complex conditions, avoiding the repetition of tasks which are needed by two conditions and providing clarity in presenting information. Case Notes’ LTC and NSF packages use Care Pathways management to manage complex conditions.
