Case Notes Single Assessment Process (SAP) package provides a single point of reference for care of the elderly across Health and Social Care settings wherever and whenever it is needed. The SAP package uses the full power of CSW’s Case Notes shared care records system to integrate information from existing systems, provide the framework for creating, updating and sharing assessments and driving the entire process using a care pathway that can be tailored to the needs of each person
Case Notes’ SAP package has been successfully deployed in Tower Hamlets and is a major milestone for the NHS Care Records Service in London as it is the first system that allows staff from the NHS and Social Services to share information on the individuals in their care.
