Case Notes may be used as a complete Care Records System for clinical management and shared care within or across settings and organisations. This applies to long-term and acute conditions and care pathways for integration of administrative, departmental or peripheral systems.
Case Notes Single Assessment Process (SAP) is an example of a Shared Care Records system which is used across multiple acute, community and social care settings, meeting specific local requirements and supporting care through the referral, care planning and care delivery processes. It meets the need for the NHS National Service Framework (NSF) for Older People in the UK. Its main components are the Case Notes Base System, Case Notes Core Services and Smart Forms module, supported by the Information Modelling ToolKit.
Case Notes for Long Term Conditions (LTC) is a further evolution of Case Notes for Single Assessment Process. Case Notes modules such as Care Planning, Care Pathways and Order Communications and Results Reporting add functionality for use within the care process, leading to a targeted and comprehensive system for multiple and complex needs.
